Migration of Legacy Data Helps Ease EHR Transition after Acquisition
The Problem
Busy Cardiology clinic on Allscripts EHR was acquired by a large healthcare organization on an Epic EHR.
Needed to migrate clinical data for approximately 10,000 active patients.
Legacy data needed to be incorporated with data preexisting in the target system.
Nearly perfect accuracy required.
Highly inconsistent source data, making it extremely difficult to attain high accuracy rates with automated conversion.
Scope included 7 data sets (allergies, medications, active problems, past medical history, surgical history, family history), plus some very detailed free-text comments.
Need to keep charts updated during the migration process, including keeping patient identifiers in sync as charts were getting merged.
Very short project ramp-up time (2 months).

Manually reviewed every data point and made the appropriate selections in Epic, resolving all exceptions while simultaneously reconciling the legacy data against preexisting data in Epic.
Used proprietary tools to prioritize charts according to daily appointments and by frequency of prior visits. Walk-in and same-day appointments migrated stat.
